All new clients must fill out First time Client Health History Form and Screening Questionnare form prior to apppointment. I will accept these by email at time of booking or any other time before appointment. Otherwise, please print and fill these out then bring to appointment. I will have these forms available to you at site of massage if you cannot bring them or e-mail them to me. Anyone completeing paperwork at site of massage please plan your time accordingly and arrive early; 10 mins is usually adequate.
